How To Register: Fee may be paid via credit card or cash and must be submitted with an online or print registration form. You may complete the online form below to register or click here to print the form. Please submit print registration forms along with payment to Five Window Beer Co., 9 W Locust St. Lodi, CA 95240. Teams are not considered registered until fee is paid.

100% of registration fees will be donated towards the Operation Puppy Drop mission.

Rules and Regulations:

• All participants must be at least 18-years-old.
• Cornhole boards, bags, and scoreboard will be provided.
• Teams may bring their own bags and boards for practice.
• Teams will be expected to keep their own score.
• Teams will play simultaneously on pre-set courts.
• Teams should not interfere with one another.
• This tournament will be double elimination. Bracket will be selected at random.
• Teams not present at 11:30AM on the day of tournament will be disqualified.
• Registration fees are nonrefundable.

• All teams are asked to be respectful of one another and other attendees. This is a family friendly event.
• No outside food or beverage inside the venue.
• Each team member plays at their own risk and by returning this registration form acknowledges that Operation Puppy Drop and Five Window Beer Co. are not responsible for any personal injury or damage to one’s property that might incur by participating in this tournament.
